When six implants earn their extra cost
More is not automatically better. Here is when it genuinely is.
A heavy bite
Bruxism and a strong masseter transfer force into the bridge that four implants can carry but six carry with more margin. If you have worn your natural teeth flat or broken a previous restoration, that history is the argument for six.
Softer bone in the upper jaw
The upper jaw is spongier than the lower. Where the CBCT shows lower density, spreading the load across six anchors reduces the force each one absorbs during the months before the bone has fully fused around them.
A longer arch
A wide jaw means a longer bridge and more unsupported span behind the last implant — the cantilever. Adding implants shortens that overhang, which is the part of the bridge most likely to flex and eventually chip.

The front of the jaw is the predictable part
Between the two mental nerves in the lower jaw, and between the two sinus cavities in the upper, sits the densest and most reliable bone in the mouth. Every full-arch design starts there, which is why the first implants of a four-implant arch and of a six-implant arch often end up in almost the same places. The difference between the two protocols is not at the front. It is everything behind it.
The back of the jaw is where the argument happens
Behind that safe zone the anatomy closes in: the sinus floor drops down from above in the upper jaw, the inferior alveolar nerve runs forward below in the lower, and the ridge that once held your molars is usually the first bone to have resorbed. A four-implant arch answers this by tilting the two rear implants forward, out of the way and into denser bone. Six implants means finding two more genuinely usable sites back there, and the CBCT either shows them or it does not.
Spread matters more than count
What holds a full arch steady is the distance between the most forward implant and the most rearward one. Six implants bunched into the front of the jaw support a bridge worse than four that are well spread, because the bridge pivots around a short base. This is the reason a good plan can end up recommending four angled implants over six upright ones, and why 'how many' is never the whole question a quote should be answering.
The teeth decide before the bone does
Positions are planned backwards from where the finished teeth need to sit, not only from where the bone happens to be thickest. An implant that emerges under the lip line, or through the face of a front tooth, forces the lab to hide it: thicker material, a screw channel in a visible surface, a shape that traps food. Six implants mean two more positions that have to satisfy the bone and the tooth line at the same time — which is why they are planned on the CBCT and placed to that plan.
What decides four or six: the scan, and what you bite against
Two jaws that look identical from the front get different prescriptions for reasons you cannot see in a mirror.

What is in the opposing jaw
A bridge that bites against natural teeth or against another fixed implant arch takes far more force than one biting against a complete denture, which yields and slides. This is why the two arches in the same mouth are often prescribed differently, and why a patient restoring one jaw against a healthy natural one is a stronger candidate for six than a patient whose other jaw is a removable denture.
Where more implants do not compensate
Heavy smoking, uncontrolled diabetes and a history of advanced gum disease all work against bone healing around an implant, and they work against the sixth exactly as much as the first. If that is your history, the useful conversation is about controlling those factors and about a plan you can maintain at home — not about buying two more anchors. Say so at the consultation. Sometimes it changes the count; sometimes it means we recommend treating something else first.
